Student Media Opt-Out Form

Parents:

We love to celebrate our students' accomplishments, which sometimes means sharing their names, photos, and achievements. Your child’s name or image may appear in school yearbooks, school or district publications, or on the school and district websites and social media pages. We may also record student performances or classroom activities and share student achievements with local newspapers, TV, and radio stations. Please be aware that this information may be accessible to the public.

If you do not want your child’s name, photo, digital image, participation in activities or sports, or achievements to be publicized (including publication in the yearbook), please complete and return the form below to your child’s school.

Important: If you sign the media opt-out form, your child’s name and image will be excluded from all school or district publications, including the yearbook and team photos. If the form is not returned, we will assume you give us permission to share your child’s name and image.
